Default Midi notes not recorded

About one in five midi notes are missed in the recording of my midi drums even though they are synthesized correctly when passed straight to a virtual instrument. It is VERY annoying. A screenshot of a few hits on a Kat pad is attached. The midi notes are recorded on the first track, and on the second track they are passed into a drum input plugin. As you can see, two notes are not recorded in the midi track (3rd and 9th) even though the are recorded in the audio track.

Default Defected to Waveform

I have stopped using Reaper because of this bug. Waveform records fine, and so does Reason.

Are you using input quantization, perhaps?

With the draconian limits on attachements (max resolution 800 x 600, max filesize 64kB) a blurry jpg is the best I can do.

The problem is simple enough though. If I record midi with an output plugin that synthesizes the audio, all the played notes are heard but around one in five is missing from the recording. It happens with a Kat KTMP1, Kat DK-10, and a Roland SPD-S, so it is not specific to the controller.

Wait, now I have played around with the settings a bit. It happens consistently when recording mode is overdub. However, it seems to work fine with plain midi recording.
