Seems like your keyboard might have a stuck control if it immediately adds itself when you try to add a shortcut. Mess around with that control to see if you can unstick it first. Otherwise, if it persists after reboot, you may need to replace it.
I would get this behavior a lot from a wireless keyboard I had and its modifier keys. They would fail to send the 'key release' message once in a while and my computer where thereafter believe they were still pressed, until I manually pressed the offending key.
Also, on a MIDI controller, some controls also keep transmitting when at the top or bottom. You might be able to fix that by opening up the controller and cleaning out the contacts with contact cleaner. A vigorous 'wiggling' of the control might get rid of the dirt as well.
