Default Multiple takes all playing, sometimes not?

This is something I never quiet worked out why. Lets say I have a track with couple of takes on it, both un-muted. When I press play, sometimes they would play both together, but sometimes only the first one would play. This only seem to change when I restart Reaper but than I gain I can't be sure because I never pay attention to when and how it happens.
I should mention that the track is NOT in "free item positioning" mode, in which I know they will always all play unless some are muted.
Can someone explain why is that happening? I'm sure it's something I do but don't know what.

It could be many things. In project preferences there is an option for overlapping media items, could it be set to "always mix" or something like that?

Maybe you have the recording option for new takes as lanes instead of takes? Takes are inside the item, so you cannot mute or unmute takes from one another. Either the item is muted in which all takes are muted, or unmuted and only one takes plays at a time inside that item.
