Short MIDI notes skipping

When I do fast 32nd or 64th notes on a synthesizer plugin, reaper sometimes skips a note... it happens rather randomly and is very unpredictable. It happens both on playback as well as rendering where notes are randomly skipped. Is there any reason what might be happening? is this reaper MIDI behaviour? I have set the attack to zero on the synthesizer plugin, so I expect immediate notes.

If a different plugin doesn't do it to the same midi item I suppose it has to be within the plugin somehow.

Not sure if this applies here, but if I use full speed render I end up missing notes with vsti's.
