Default fixing "stuck notes", no note-off

Not sure what the cause of this is, whether it be my sequencer (TidalCycles), DAW (Reaper), or VSTs (U-he Diva), but sometimes when I'm playing notes, I get "stuck notes" where it seems no note-off is sent and it just plays indefinitely.. Is there any way on the Reaper side of things to mitigate this?

i keep the send all notes off action on a toolbar button as well as on my midi controller. lot of different things can cause this.

Try setting the MIDI input to a specific channel if it's set to All Channels.
