I'd go along with the premise that REAPER should always ask what to do when you insert Melodyne because we can face all these scenarios.
In all these scenerios if you've just added Melodyne the project should play exactly as it did before Melodyne was added until you do editing in Melodyne. I don't know how easy that would be but an editor shouldn't do anything ever until you instruct it to edit something. If it might change something by just adding it, it should stop and ask what you want to do first.
